EM Literature of Note
FEBRUARY 3, 2025
Its been a long haul for Emergency Medicine Literature of Note and, as AI becomes a greater portion of my professional life, correspondingly, the time shrinks for a long-form Emergency Medicine post. So, in an experiment, Ive created and migrated to a more-modern platform the Medicine Minute Substack. The hope is, with short format thoughts, there can be more-frequent updates and the longer rants can still have either a life here, or in ACEPNow.

A Google search brings up news releases from Vertex , and news coverage—which are essentially the same and lacking links to the evidence. I aimed to provide readers the evidence for this new drug. This was much more difficult to find. Some things to set out first: I am not a pain specialist; my analysis is one of a Neutral Martian.

Background: ST-segment elevation myocardial infarction (STEMI) is a critical condition requiring rapid diagnosis and intervention. While ECGs play a central role in detecting STEMI, misinterpretations often occur, leading to unnecessary catheterization or missed diagnoses. With advancements in artificial intelligence (AI), there is potential for machine learning models to assist in more accurate detection of STEMI.

These can lead to a wide spectrum of emergent complications including but not limited to cardiac dysrhythmias, seizures, and acute renal failure. In this post, we will focus on the risk stratification of these patients, the mechanism of TLS in the context of electrolyte changes, and how to manage the emergent consequences of these derangements ( 1,2).
