An immediate 12-lead EKG was obtained: There is ST elevation in leads aVR and V1, with marked ST depression in I, II, III, aVF, V3-V6. DISCUSSION: The 12-lead EKG EMS initially obtained for this patient showed severe ischemia, with profound "infero-lateral" ST depression and reciprocal ST elevation in lead aVR. What should be done?

She is concerned about food poisoning and says the groin/thigh pain feels like a prior episode of sciatica. EKG shows atrial fibrillation with a rate of 169. She had no persistent vomiting, was not dehydrated and had no fever. It began 5 days earlier after eating chicken nuggets from Wendy’s. Temp is 98.7 and pulse 121.

Jeff: For any patient arriving with suspected cannabis or synthetic abuse, consider checking an EKG. Be sure to look for and treat dehydration, acute renal failure, and rhabdo though.
