1 Seizures may occur due to lowered seizure threshold. 1,2 Neuroleptic malignant syndrome (NMS) (hyperthermia, autonomic instability, rigidity, altered mental status [AMS]) can occur as well and is most often seen with clozapine but has been observed with other atypicals. 6 Seizures have been observed and are dose-dependent.

The patient had a witnessed generalized tonic-clonic seizure leading to GCS 4. In the resuscitation room, the patient had another seizure that stopped after IV Lorazepam. Hypothermia can also produce bradycardia and J waves, with a pseudo-STEMI pattern. EMS reported an initial GCS of 8 with pupils equal and reactive.

There is weak evidence to suggest that moderate hypothermia (32-33 o C) can be used for ICP control in specialist settings. Do we need seizure prophylaxis? Children and young people have lower seizure thresholds than adults. Either levetiracetam or phenytoin can be used for seizure prophylaxis. Published 2022 Nov 20.
