We’ll keep it short, while you keep that EM brain sharp. A 34-year-old male is brought via EMS after collapsing during an outdoor adventure race. EMS reports the patient was conscious but altered, with slurred speech and confusion. Point of care glucose in the field was 102 mg/dL. 1 Fever is usually < 40C.

EMS found the patient in VFib and performed ACLS for 26 minutes then obtained ROSC. The patient received therapeutic hypothermia at 33 degrees C for 24 hours. Case submitted by Magnus Nossen MD from Norway, written by Pendell Meyers A man in his 50s with no pertinent medical history suffered a witnessed cardiac arrest.

That write up was for the DOSE-VF pilot trial done in March 2020 and can be found here. Within the United States, due to paramedic shortages, this dual-medic setup does not exist in many EMS systems.

Case submitted and written by Dr. Mazen El-Baba and Dr. Evelyn Dell, with edits from Jesse McLaren EMS brought a John Doe, in his 30s, who was found in an urban forest near a homeless encampment on a cool fall day. EMS reported an initial GCS of 8 with pupils equal and reactive. There were no signs of trauma on scene or on the patient.
