Neonates presenting to the Emergency Department often cause a lot of uncertainty. We have previously discussed hypothermia presentations , GBS infections , and HSV infections. 2019, there were 1,875 and this trend has continued to increase (2,157 case in 2020; 2,855 cases in 2021). doi: 10.1136/bcr-2020-240532.

Previous triad of death = hypothermia, acidosis, and coagulopathy. Liver dysfunction, secondary cirrhosis, critical illness, trauma, and hypothermia reduce citrate metabolism. Hypothermia also leads to hypoCa. Today on the emDOCs cast with Brit Long, MD ( @long_brit) , we look at hypocalcemia in trauma and the diamond of death.

3-6 Clinical Presentation: Symptoms include altered mental status, tachycardia, hyperthermia, urinary retention, mydriasis, blurred vision, dry skin, hallucinations. Induced hypothermia after cardiac arrest is also called “active temperature control” or “targeted temperature management.” Temperature management (or fever avoidance) should begin immediately after cardiac arrest (ideally in the emergency department, for out-of-hospital arrests) and continued for at least 72 hours.

1,2 Neuroleptic malignant syndrome (NMS) (hyperthermia, autonomic instability, rigidity, altered mental status [AMS]) can occur as well and is most often seen with clozapine but has been observed with other atypicals.
