Nachi: All unstable patients or those with suspected or proven ectopic or heterotopic pregnancies should be immediately resuscitated and taken for surgical intervention. A 2015 randomized trial found similar outcomes for IM methotrexate compared to placebo for tubal ectopics. Initial study conclusions are promising.
