Circumflex Occlusion May be Subtle or Invisible on the ECG

reports MI in 2001 with a stent placed in the "marginal" artery. Here is the EKG at 1810: A repeat ECG 1 hour later is subtly changed, with some evolving T-wave inversion best seen in V5 and V6. Angiography and PCI were undertake 8 hours after the initial ECG and showed a completely occluded OM-1. Exam is unremarkable.

Acute OMI or "Benign" Early Repolarization?

Right precordial ST depression in a patient with chest pain

This is his 12-Lead ECG: What do you think?​ Firstly, subendocardial ischemia does not localize on 12-Lead ECG. The precordial ST-depression pattern on this ECG (and in this clinical setting) should immediately raise suspicion of Posterior STEMI! What's also interesting to note is that there is ST-elevation in V6 on the ECG.

If a wide complex rhythm is present on ECG, sodium bicarbonate boluses (to overcome sodium channel blockade) should be administered. 2001 Nov; 9(8):565-74. Treatment: The mainstay of treatment is supportive care. Seizures should be treated with benzodiazepines. Cardiac dysrhythmias should be treated in the standard fashion.
